What Your Notice Must Include

3. Identification of the material claimed to be infringing and information reasonably sufficient to let us locate it (e.g. a room code or URL).
4. Your contact information — address, telephone number, and email address.
5. A statement that you have a good-faith belief that the use is not authorized by the copyright owner, its agent, or the law.
6. A statement, made under penalty of perjury, that the information in the notice is accurate and that you are the copyright owner or authorized to act on their behalf.

If you believe material you posted was removed or disabled by mistake or misidentification, you may submit a counter-notification to the Designated Agent above containing the information required under 17 U.S.C. § 512(g)(3).

We will terminate, in appropriate circumstances, the access of users who are determined to be repeat infringers.
